What’s the difference between medical abortion and surgical abortion?

In Australia, around one in four people who are able to get pregnant will have a medical or surgical abortion in their lifetime.

Both options are safe, legal and effective. The choice between them usually comes down to personal preference and availability.

So, what’s the difference?

What is a medical abortion?

A medical abortion involves taking two types of tablets, sold together in Australia as MS2Step.

The first tablet, mifepristone, stops the hormone progesterone, which is needed for pregnancy. This causes the lining of the uterus to break down and stops the embryo from growing.

After taking mifepristone, you wait 36–48 hours before taking the second tablet, misoprostol. Misoprostol makes the cervix (the opening of the uterus) softer and starts contractions to expel the pregnancy.

It’s normal to have strong pain and heavy bleeding with clots after taking misoprostol. Pain relief including ibuprofen and paracetamol can help.

After two to six hours, the bleeding and pain usually become like a normal period, although this may last between two to six weeks.

Haemorrhage after a medical abortion is rare (occurring in fewer than 1% of abortions). But you should seek help if bleeding remains heavy (if you soak two pads per hour for two consecutive hours) or if you have have signs of infection (such as a fever, increasing abdominal pain or smelly vaginal discharge).

Do I have to go to hospital?

It is legal to have a medical abortion outside of a hospital up to nine weeks of pregnancy.

Depending on state or territory law, the medication can be prescribed by a qualified health-care provider such as a GP, nurse practitioner or endorsed midwife. These clinicians often work in GP surgeries or sexual and reproductive health clinics and they may use telehealth.

Medical abortions also occur after nine weeks of pregnancy, but these are done in hospitals and overseen by doctors alongside nurses or midwives.

Medical abortions after 20 weeks are done by taking medications to start early labour in a maternity unit. Often, medications are first given to stop the foetal heartbeat so it is not born alive. Then, other medications are given to manage pain.

These types of abortions are very rare. They may be used when an obstacle has prevented someone accessing an abortion earlier, continuing with the pregnancy is dangerous for the pregnant person’s health or if there is a serious problem with the foetus.

What is a surgical abortion?

Surgical abortions are performed in an operating unit, usually with sedation, so you will not remember the procedure. Surgical abortions are sometimes preferred over medical abortions because they are quicker. But the decision should be between you and your health-care provider.

In the first 12–14 weeks of pregnancy, a surgical abortion takes less than 15 minutes and patients are usually discharged a few hours after the procedure.

Medications may be given before surgery to soften and open the cervix and to ease pain. During the procedure, the cervix is gently stretched open and the contents of the uterus are removed with a small tube. This procedure is carried out by trained doctors with the assistance of nurses.

Surgical abortions after 12–14 weeks are more complex and are performed by specially trained doctors. Similar to medical abortions, medications may be given first to stop the foetal heartbeat.

It is normal to experience some cramping and bleeding after a surgical abortion, which can last about two weeks. However, like medical abortion, you should seek help for heavy bleeding or signs of infection.

Do I need an ultrasound?

It used to be common before an abortion to have an ultrasound scan to check how far along the pregnancy was and to make sure it was not ectopic (outside the uterus).

However, this is no longer recommended in the early stages of pregnancy (up to 14 weeks) if it delays access to abortion. If the date of the last menstrual period is known and there are no other concerning symptoms, an ultrasound scan may not be necessary.

This means people can access medical abortion much sooner, even from the first day of a missed period, without waiting for the embryo to be big enough to be seen on an ultrasound scan. This is called “very early medical abortion”.

Before and after care

Before having an abortion, a health-care provider will explain common side effects and when to seek urgent medical attention. For people who want it, many types of contraception can be started the day of abortion.

Even though the success rate of medical abortion is very high (over 95%) it is routine to make sure the person is no longer pregnant.

This is usually done two to three weeks after taking the first tablet mifepristone, either by a low-sensitivity urine pregnancy test (which you can do at home) or a blood test.

In the rare case a medical abortion has not worked, a surgical abortion can be done.

Sometimes after a medical or surgical abortion, tissue is left behind in the uterus. If this happens you may need another dose of misoprostol (the second tablet) or a surgical procedure to remove the tissue.

Some people may also seek support-based counselling or peer support to help them work through the emotions that might accompany having an abortion.

Understanding the differences and similarities between medical and surgical abortions can help individuals make informed decisions about their reproductive health.

People with brain injuries have a high risk of romance scams. ‘Scambassadors’ can help shed stigma

Romance scams – where scammers create fake identities and use dating or friendship to get your trust and money – cost Australians A$201 million last year.

But the emotional impact of romance scams can often feel worse than losing money. Those who have been scammed may experience shame and embarrassment and have difficulty accepting the relationship wasn’t real.

People who have acquired a brain injury, for example after a stroke or car accident, may be more vulnerable to these scams. My research with colleagues shows they are often less aware of scams and find it harder to recognise red flags.

But our project has found there are benefits when people with brain injuries who have been scammed share their experiences. It can create awareness, reduce stigma and help prevent future scams.

Some groups are more vulnerable

Anyone can be scammed. But some groups are more at risk, including people with a disability such as an acquired brain injury.

We surveyed 101 clinicians in Australia and New Zealand who work with people who have acquired brain injuries. More than half (53%) had a client who had been affected by a cyberscam. The most common type was a romance scam.

How do romance scams work?

Romance scams involve a scammer (or sometimes multiple people) luring someone into a fake relationship in order to exploit them, often to get money. Scammers may use online dating platforms to connect, or social media, gaming and even online shopping sites.

Romance scammers build trust and strong emotions using techniques such as love bombing (early and frequent declarations of affection), grooming and manipulation over an extended period of time. They share common interests and even similar types of trauma to make people trust them. As a romance scam survivor with an acquired brain injury explained:

My way of thinking was sort of skewed because all I seen was love, the money, all the things I wanted, so I didn’t worry about all the other little stuff.

The identity of the scammer usually appears very attractive and trustworthy but is often fake, stolen from a real person or AI-generated. They present lots of evidence and exciting details about their everyday life to appear real and keep people hooked into the relationship.

Scammers use pre-written scripts with compelling narratives describing significant financial success, being a widower or orphan, or working overseas to attract people. Flirty language and flattery makes people want to keep communicating with the scammer. They might tell you they think you have a beautiful smile and their dog or cat would love you.

Scammers will invest weeks and months to build up a connection, then scammers present exciting “opportunities”. These may include investments and requests to cover international flights for a first meet-up. Or paying for medical bills for a sick relative. As a scam survivor with an acquired brain injury explained:

She was really jumping into a kind of quite intimate relationship with me, even though we haven’t met yet, but she’s promising we will one day. All I need to do is send money.

Why are people with brain injuries more at risk?

One in 45 Australians lives with a brain injury acquired during an event such as car accident or stroke. This can damage a specific part of the brain, widespread brain cells (neurons), or both.

The impact of a brain injury varies but can affect cognition, emotions, behaviour and neurological functioning. As a result, people can experience changes in their ability to care for themselves, work, socialise and make complex decisions.

Cognitive difficulties – such as memory problems and reduced information processing – can make it harder to learn, notice and respond to scam red flags in real time. People may struggle to comprehend new or complex information, have reduced judgement and be more impulsive.

Like other vulnerable Australians, people with brain injury may also be bored, lonely and require care. This may mean the scammers’ constant online availability, messaging, attention, praise and acceptance of the person regardless of their disability is even more attractive.

Shame and embarrassment

People with acquired brain injuries may also struggle to move from knowing or intending to do something, to actually enacting that behaviour. This is called the frontal lobe paradox. As one of our interviewees explained:

I make some big realisations and then I forget about it, like … ‘don’t do that again’. And then I go and do it again.

When the scam is uncovered, family, friends and frontline services such as police and banks may respond with blame, judgement and ridicule. This contributes to further distress:

They just say how stupid I was for being conned.

Scams are likely under-reported to authorities such as Scamwatch as a result of shame and a lack of awareness about scams.

Family and clinicians of people with brain injury may cut or reduce their access to money or the internet, which adds to the scam’s financial and emotional impacts.

As one clinician explained:

The depression […] didn’t come from being scammed. It came from quite the opposite, almost like he […] feels like it’s his right to have access and leave himself open to those things.

Authentic partnerships with people with lived experience

Our research team has developed a suite of tailored resources called CyberAbility, which were co-designed with people who have brain injuries and experience of being scammed. We call them “Scambassadors”.

The Scambassadors alleviate some of the stigma and shame associated with being scammed, through conducting community education sessions, speaking with media, and co-facilitating therapy groups. They also spread awareness to other people with brain injuries about what to look out for – but the advice is helpful for everyone.

You can look out for signs someone in your life is being scammed. This could include a new unmet lover, major unexpected purchases or loans, or an increase in secrecy about online activities.

Approach difficult conversations about possible scams with curiosity, care and patience. Share your own experiences of being scammed or ripped off to normalise this and reduce judgement.

Whoever is scammed, the real fault lies with the criminals who commit this fraud. But regularly discussing scams can help reduce stigma and protect our community.The Conversation

Children can learn to eat new vegetables if they are introduced regularly before the age of two, suggests a new study. Even fussy little eaters can be encouraged to eat more greens if they are offered them five to 10 times, University of Leeds study found.
Olga Yazhgunovich: Exposing babies to a new vegetable early in life encourages them to eat more of it compared to offering new vegetables to older children, Researchers from the University of Leeds say. Mothers and fathers shouldn't let facial expressions discourage them either when children are tasting foods for the first time, according to researcher Marion Hetherington. "You have to get beyond those facial expressions of what you think might be disgust, but are actually just shock," Hetherington said. "...Be patient and be persistent, but don't give up after three tries." The research team gave artichoke puree to 332 children aged between four and 38 months from the UK, France and Denmark. Globe artichoke was chosen as the vegetable in the study because parents said it was one of the vegetables they were least likely to cook. One in five cleared their plates while 40% learned to like artichoke, BBC reports.Twenty-one per cent consumed more than three-quarters of their serving each time and were labelled "plate-clearers". "Non-eaters" made up 16% of the children because they ate less than 10g even when it was offered for a fifth time, while the rest did not conform to any one group. The study also dispelled the popular myth that vegetable tastes need to be masked in order for children to eat them. The puree was either served straight, or sweetened with added sugar, or vegetable oil was mixed into the puree to add energy. The researchers found there was little difference in the amount eaten over time between those who were fed the basic puree and those who had the sweetened one, suggesting that making vegetables sweeter does not encourage children to eat more. Overall, they did find that younger children ate more artichoke than older children in the study. Prof Marion Hetherington, study author from the Institute of Psychological Sciences at Leeds, said this was because children become picky and wary at a certain age. "If they are under two they will eat new vegetables because they tend to be willing and open to new experiences.After 24 months, children become reluctant to try new things and start to reject foods - even those they previously liked." Prof Hetherington said her research, which is published in the journal PLOS ONE and funded by the EU, offered some valuable guidance to parents who want to encourage healthy diets in their children. Washington: If you an animal companion in your home, be careful, as a new study shows that household pets can transmit infection to people, and make them sick. People with weak immune systems, young children, pregnant women and seniors are especially vulnerable, and health care providers and pet owners should be aware of this risk to prevent illness. Dr. Jason Stull, assistant professor at Ohio State University said according to surveys, the general public and people at high risk for pet-associated disease are not aware of the risks associated with high-risk pet practices or recommendations to reduce them. All pets can transmit diseases to people. For instance, dogs, cats, rodents, reptiles and amphibians can transmit Salmonella, multi-drug resistant bacteria (including Clostridium difficile), Campylobacter jejuni and other diseases. Parasites such as hookworm, roundworm and Toxoplasma can also be transmitted. Infection can be contracted from bites, scratches, saliva and contact with feces. Reptiles and amphibians can transmit disease indirectly, such as via contaminated surfaces. For healthy people, the risk of pet-associated disease is low, but vulnerable people are at risk, including newborns, children with leukemia and adults with cancer. Dr. Stull said that patients at high risk and their households should have increased vigilance of their pets' health and take precautions to reduce pathogen transmission. Recommendations for reducing transmission of infection include: 
  • Wearing protective gloves to clean aquariums and cages and remove feces 
  • Proper hand-washing after pet contact
  • Discouraging pets from face licking
  • Covering playground boxes when not in use
  • Avoiding contact with exotic animals
  • Regular cleaning and disinfection of animal cages, feeding areas and bedding
  • Locating litter boxes away from areas where eating and food preparation occur
  • Waiting to acquire a new pet until immune status has improved
  • Regularly scheduling veterinary visits for all pets.
Physicians and other health care providers should enquire about pets and repeat questions in light of illness in vulnerable people, as well as advise on the risks of pet ownership and how to reduce risks of disease.
